How do I get copies of my medical records?

Patient may visit The Medical Records Department during business hours to request copies of their Medical Records.
As an alternative to coming to the department, you may download and complete the form listed here and mail to the Medical Records Department along with a copy of your Driver’s License or Picture Identification.
Normally, there is a charge for copies of medical records and we can usually process the requests within three working days of receipt of the request.
